YES I/DD PEER SUPPORT SERVICES

The YES I/DD PSS Program serves individuals ages 6-55 who have been diagnosed with Intellectual Developmental Disability (I/DD) or who, before the age of 22, received a Traumatic Brain Injury (TBI) diagnosis and who also have a co-occuring Severe Mental Illness (SMI) or Severe Emotional Disorder (SED) diagnosis such as a Mood Disorder or Depression.  We offer FREE Peer Support Services for up to 3 hours per week provided by an assigned Peer Support Specialist for 90 days.  Our Peer Support Services are structured, scheduled interventions offered in the community for people ages 6-55. We address the person’s identified needs and connect them to Long Term Care Services.
